PRACTICE EMERGENCY CLOSURE

The practice will not be open on 9th June 2010 due to a major flood of the premises. There will be staff available to see emergency patients only. All routine appointments are cancelled. We sincerely apologise for any inconvenience this may cause. Please call the surgery for further information.

Cancelling your Appointment

If you are unable to attend an appointment with one of the doctors or nurses, please telephone or use the link at the bottom of this page to cancel your appointment.

By giving us as much notice as you can you are helping us to make sure that someone else is given your slot.

When the practice is closed

If you need urgent help or advice there is always a doctor on-call. Telephone the Surgery number and you will be told the number to call for help. Have a pencil and paper ready to write down the number and enough coins to make another call if you are phoning from a call box.

New Patient Registration

Registration forms are available from our Reception Team, or if you wish go to the next tab on this page and follow the instructions to pre-register, that is to fill out the form online and sign it when you first visit the surgery.

At the time of registering you will be invited to attend for a basic health assessment carried out by a Practice Nurse or Healthcare Assistant. The purpose of this is to give the practice some background information on your current health before your complete medical record is received from your previous GP.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
